Role request with future start date: Task not being created

I created a request for an Enterprise Role for a user on November 15. In the request, I set the start date to November 16, 8:00 am, and the end date to November 17, 08:00 am.

We have an EnterpriseRoleManagementJob which runs once per hour, but even after the start date has passed, no task has been created.

In the Global Configuration under Tasks, we also tried setting "Create Task if Start date less than" to 1 min, but to no avail.

The user, account and role are all active and work for other cases. The role has been assigned successfully in the past.

Is there anything else we can check to see where the problem lies?

yes, it is scheduled and running, still, the task has not been created.
