When building Custom Application I'm receiving an error on enrollment.

I receive an error when attempting to create a custom application that targets the Saviynt Manual Tasks workflow.    When completing enrollment it gives me the following error:

Anyone have any thoughts on how to get through this?

@dahl , Are you getting this error on click of Create option under Custom Application. 

Please share the steps and sample accounts file used for onboarding.
