while submitting the request through ARS for Disconnected application can we add attachment form,

Like if user select that application add to cart and then while selecting roles, user can see on option of attachment file there and he can open that and fill required information before submitting.

There is an option to attach a file before submitting the request in the comments section. Is that not able to meet your requirements? Could you please elaborate more on the exact requirement.

Like if we want to add document form which user can download from passport and fill the information and submit back.

Currently this is not supported. We cannot attach a form which user can download, fill and attach back.
