Workflow triggering "if" true and false condition at once

We have the next workflow: 

And we are receiving both email templates (marked with yellow in the image) at the same time, in the same trigger of the workflow. Any idea why this is happening? Thank you in advance.

• what is type of request 
• provide extract of query select * from request_Access where requestkey =1111 (update request key)
