Workflow creating 2 Requests

Hi Experts , 

I created one workflow like the below : 

so what's happening right now if i request entitlement b and account a , it is creating 2 request id for the same . I mean the approver has to approver 2 times for this request one for account and another for entitlement 

I want that only one request should get created for one endpoint and entitlement .

Does 2 request are created 1 for account and another for entitlement ?

What is end requirement you want to achieve ? Please share request screenshot and confirm what are values of endpoint cp45 & ent cp 40

Yes , it is getting raised1 for account and another for entitlement . The endgoal is to have one request with child entries inside . 
The value of cp45 or cp40 decides the level of workflow here 2 level , 3 level or autoapprove .

The user should see the parent request and inside he should see the embedded add access request to approve or reject in one go.
