Need to get the records where the certifier has "terminated" the accounts

Hi Team,

We have a requirement to configure an user manager campaign with only the Employment Verification step. The certifiers can choose options "Works for me","Does not works for me" or "Terminated".

We want to create an analytics report which will fetch the records which were terminated in that particular campaign.

Can someone please help me with the query.
