multi user bulk request through csv

I am trying to upload add access to multiple users using csv method. It is working fine through manual process but using csv method it is not adding access to multiple users. Below is the format. Does the format looks fine ?

• If its AD then entitlement type must be memberOf
• Attach email template in global configuration which will send email for request creation failure.

Have you configured workflow in global configuration for bulk upload? If not, please assign auto approval workflow and try..
