User Update Request form

Hello! We're trying to configure the User Update request form but we are getting these error popouts. 

So far, I think this is the log we think is related to that error:

Hello! Here are the attributes:

Is there any action string added ?

keep users column in lower case like employeeID to employeeid

Yes. We need an action string to make the end date required/not required based on the employee type. modifying the users column didn't change anything :'(

Hide employeeclass & employeeType field & try loading request form

I was just exploring this forum. Could you please tell me where can i find this form once created. ex: I have done ARS form that we can find while requesting access . likewise where can we find user update form once created.?

You can find under ARS -> Create/ Update User Request tile.

Hi, @rushikeshvartak Thank you for the responses to this issue. This has been resolved. It was the code in the manager attribute that causing the issue.

What turned out to be the error in the manager field?  I have a very similar error at my end and a similar manager sql statement.
