How to get Entitlements having role owner information with other entitlements

I want to extract all the entitlements of an endpoint EP with role owner information & other entitlement mappings also. Please help with SQL query.

Provide screenshots for your requirements as you are mentioning entitlement and role owner which does not have correlation

Example - There is an Endpoint EDP1, it has entitlement type Groups with entitlement named User_ReadOnly this entitlement has owner "Kiran Kumar". in Other Entitlement Details it has one AD group Mapped "CN=test_db_datawriter,OU=TestDatabaseGroups,OU=ABC,DC=ABC,DC=test,DC=com".

I need to get the outpout od an SQL query as below - 

Application Name	EntitlementType	Entitlement_Value	Entitlement_Owner	Other  Entitlement Details
EDP1	Groups	User_ReadOnly	Kiran Kumar	CN=user_readonly,OU=TestDatabaseGroups,OU=ABC,DC=ABC,DC=test,DC=com
