Getting Error when submitting second requests for Disconnected application

Hi,

I have a requirement to create a plain ARS request form with only one Dynamic attributes on the form and No entitlements. It is a Disconnected application and we only have to send out a mail through Saviynt at the time of request submission & rest of the processes are manual from application team side. so, for Saviynt, It's just a fire and forget.

I created an endpoint and tried raising a request. I am able to submit the request for a new user but once that request is completed and I try to raise a request for the same user again (by selecting other attributes from the drop down) - It throws an error as soon as I hit the submit button.

The requirement is - a user should be able to raise multiple request. please note - we have only one attribute drop down field on the form but no entitlements or tables. I am attaching logs.

I tried with simple auto approve work flow also but it is still throwing same error.
