Does anyone have any ideas on how to fix the following error we are getting on Add Access requests in AD?

null Error while ADD operatio <span title='null Error while ADD operation for account-413251 to Group-CN=GroupName,OU=OUName,DC=DCname,DC=local in AD - [LDAP: error code 68 - 00000562: UpdErr: DSID-031A11C4, problem 6005 (ENTRY_EXISTS), data 0

The user appears to already be in the AD group in question. We don't have this issue on all AD accounts, in our logs you can see the same user with Add Access to AD groups they are already in that resolve to "No Action Needed."

These are cluttering up our Pending Tasks and keep trying over and over again.
