Requestform Valid Values

I am looking for a listing of the entitlement_types.requestform valid values and descriptions.  The database table field stores integers but the drop down on the entitlement type tab on the entitlement includes a list of values.

This has been a struggle in general.  Rather than inferring the mapping of the ID on the database to a value, is there fieldvalue data stored anywhere?

Note : Updated from UI and found all , Its missing in documentation
