Unable to create the AD Account

Hi,

We have recently created Account Name rule for AD application and when we create the one user the provisioning tasks getting created and processed successfully and again if we create the user with the  same first name  and last name the provisioning tasks not getting created. and got the below logs. Can any one help me on this.

Please replicate the issue. Create two users with same firstname and lastname and share the logs only for that duration for both cases. Also share the firstname and lastname of the user.
