Some help App On-Boarding Setting up Questionnaire

Team,

we are planning to build an application On-boarding template in saviynt. Need some help on Setting up Questionnaire.

The document in saviynt portal was not that explanatory or useful in my case. Here I would like to ask how can we make a questionnaire for below use case

what is the application type?

AD/DB/Rest(should be drop down list and in which variable I should store the values to display)

Sample template which we follow but would like to create a similar in saviynt

It is similar like creating dynamic attribute under Endpoint

@rushikeshvartak  The questionnaire comes to picture at the time of app on-boarding. If End use says a new endpoint,How this will apply?

Do you have any sample created for any customer in-order to refer.
