Service Account request is not showing 'Account Type' and 'Added Owner' even though it is mandatory

Service Account request is not showing 'Account Type' and 'Added Owner' even though it is set as mandatory. So the request is going to admin since the UserOwnerKey or the UserGroupOwnerKey is not getting populated. 

Please see the attachment below. I can see the highlighted fields in yellow (Account Type, Assignee and Add Owner (User) in DEV but not in Prod. What configuration do I need to change to have the Request Page show those attributes

There is no config as such. Does product version is same in DEV vs PROD ?
