Same Identity appearing multiple times in Duplicate Identity dashboard

Hi Team,

We are facing an issue where the same identity is appearing twice in the duplicate identity dashboard which shouldn't be the case. 

We have multiple occurrences of the similar issue across the environment.

Did anyone else face such issue and any solution for the same?

This is the Coarse Filter :

t1.preferedfirstname=t2.preferedfirstname and t1.customproperty14=t2.customproperty14 and t1.customproperty3=t2.customproperty3 and t1.USERKEY <> t2.USERKEY and t1.customproperty3 is not null
