Request ID is not getting created for Access Request

While testing the Active Directory application access request part , we have observed that the end user is able to select the entitlements and the required details from saviynt EIC but when he submit the Access request the Request ID is not getting created and task is not getting triggred . we are not able to see the request details under the Request history tile. 

Please find the attached Screen shot for reffrence.

Can you check if a workflow is defined in the Security system. If not, please add a workflow for Add and Remove Access.

We have already configured the Workflow in AD security system .

Is it happening for all entitlements or certain? do you have owners associated with these entitlements?

Can you share the workflow logic?

Its happening for all the entitlements , please find below the workflow details :

You mentioned entitlements but in workflow I see you have used Resource Owner Approval block. for sending approvals to entitlement owners you need to use Access Approval block. Can you modify this and check?

Also can you confirm that you have owners assigned to all the requestable entitlements?
