Evaluating Risks window Pop-ups up on Add access request

We've recently found that, despite SOD not being configured, an Intermittently Evaluating Risks window appears while requesting the access.

Unfortunately, unable to reproduce the issue and capture the logs.

Has anyone else run into this problem?
